Roll out set for standard Part D form
An AMA Workgroup and America's Health Insurance Plans (AHIP) have reached agreement on a standard form for physicians to obtain formulary exceptions and prior authorizations. For the vast majority of drugs, this one-page form will replace the often lengthy forms currently required by Part D Plans. The AMA and AHIP will work with CMS to roll out widespread use of the form as quickly as possible.

Provider Part D - CMS Weekly Conference Call
CMS will be hosting a weekly conference call on Provider Part D.
This call will be scheduled every Tuesday from 2PM - 3PM EST, beginning January 3, 2006.
