IMS JOB BANK Guidance for Lost or Destroyed Medical Records

Recent natural disasters in Iowa may have resulted in lost or destroyed patient records. The Iowa Medicaid Enterprise (IME) has provided an Informational Letter to Iowa providers regarding records lost or destroyed as a result of a disaster. An informational letter provides general guidance in this area. If you have lost medical records, you must complete an IME Attestation of Medical Record Loss or Destruction form as soon as possible. You must retain this attestation in place of the missing medical record.
